All about Macaulay impingement of the hip

Macaulay impingement, often referred to in the context of hip disorders, is a condition characterised by the abnormal interaction between the femur and the acetabulum, leading to pain and restricted movement. Named after Dr Macaulay, who first described the phenomenon, this condition primarily affects athletes and individuals engaged in activities that require repetitive hip flexion and rotation.

The underlying cause of femoroacetabular impingement typically involves structural abnormalities in the hip joint. These can include a cam-type deformity of the femoral head or a pincer-type deformity of the acetabulum. In cam impingement, the femoral head is not perfectly round, which can lead to early contact with acetabular rim during movement.

Conversely, pincer impingement occurs when there is excessive coverage of the femoral head by the acetabulum, resulting in similar early contact between the two bones. Between the two bones is a fibrocartilage structure which lines the circumference of the acetabulum, the acetabular Labrum.

Due to repeated damage in femoroacetabular impingement, patients complain of pain and a clicking sensation when the labrum is torn. Labral tears are detected on an MRI scan and can be repaired during arthroscopic surgery.

Symptoms of Macaulay impingement often manifest as groin pain, stiffness, and a decreased range of motion in the hip joint. Patients may experience discomfort during activities such as running, squatting, or even prolonged sitting.

As the condition progresses, it can lead to more severe complications like labral tears or early-onset osteoarthritis. Diagnosis typically involves a combination of physical examinations and imaging studies such as X-rays or MRI scans.

These diagnostic tools help identify any structural abnormalities and assess the extent of damage to surrounding tissues. Treatment options for Macaulay impingement vary based on severity but generally start with conservative measures. Physical therapy aimed at strengthening hip muscles and improving flexibility can be beneficial.

Non-steroidal anti-inflammatory drugs (NSAIDs) may also be prescribed to alleviate pain and inflammation. In cases where conservative treatment fails to provide relief, surgical intervention may be necessary. Procedures such as arthroscopy can be performed to reshape the femoral head or acetabulum, thereby reducing impingement and restoring normal function. When the articular cartilage is damaged and arthritis has set in, a hip replacement is advised.
