ADHD assessment: What does it involve?
ADHD (attention deficit hyperactivity disorder) is a neurodevelopmental disorder that affects behaviour and disrupts daily life. Receiving an accurate diagnosis is crucial to managing ADHD effectively, and the assessment process is an essential first step.
Dr Ana Tsakiri, leading consultant child and adolescent psychiatrist, explores the key components of an ADHD assessment, and the importance of early and accurate diagnosis.

What are the signs of ADHD?
ADHD is characterised by a persistent pattern of inattention, hyperactivity, and impulsivity that interferes with daily functioning:
- Signs of inattention may include difficulty focusing on tasks, frequent forgetfulness, being easily distracted, and struggling to follow instructions.
- Signs of hyperactivity may include restlessness, excessive talking, difficulty managing time, and an inability to sit still.
- Signs of impulsivity may include interrupting conversations, difficulty waiting their turn, acting without considering the consequences.
What does an ADHD assessment involve?
An ADHD assessment is a comprehensive evaluation conducted by a specialist, such as a psychiatrist, to gain a detailed understanding of the patient’s symptoms, history, and behaviour patterns. The goal is to determine whether the patient meets the diagnostic criteria for ADHD.
Medical history and questionnaires
The initial phase of the ADHD assessment focuses on the patient’s medical history. This involves questions about the patient’s development, family history of mental health conditions, and any past or current indicators of ADHD. Input from parents/caregivers and teachers will also be requested.
Standardised questionnaires or rating scales will also be employed to assess the severity and frequency of signs of ADHD. These tools will enable the specialist to compare the patient’s behaviours to established diagnostic criteria, such as those outlined in the DSM-5 (Diagnostic and Statistical Manual of Mental Disorders).
Observation and behaviour analysis
For children, direct observation in school or at home will also be performed to evaluate how symptoms present in different environments. The specialist will also look for consistent patterns indicating that ADHD symptoms have been present since early childhood.
Cognitive and psychological testing
In some cases, the ADHD assessment will also include cognitive or psychological tests to rule out other conditions or better understand the patient’s cognitive functioning. For example, tests assessing attention span, memory, or executive function can help differentiate ADHD from other conditions with similar symptoms, such as anxiety, autism, or learning disabilities.
How long does an ADHD assessment take?
A standard assessment session typically lasts 1 to 3 hours. However, the full diagnostic process may span several weeks or months and involve multiple appointments, depending on the complexity of the case and the depth of information needed to make a diagnosis.
If your child shows symptoms consistent with ADHD, seeking a professional assessment is essential. Although an ADHD assessment may be thorough and time-intensive, it provides valuable insights that pave the way for effective support and treatment.
What happens after an ADHD diagnosis?
ADHD treatment plans typically combine medication, therapy, and lifestyle adjustments. Medications, including stimulants like methylphenidate or non-stimulants like atomoxetine, can help regulate brain activity, improving focus and reducing impulsivity.
Therapeutic interventions, such as cognitive behavioural therapy (CBT), can help patients develop strategies to manage their symptoms effectively. Therapy is particularly helpful for addressing associated issues like low self-esteem, anxiety, or difficulties with time management.
For children and adolescents, an ADHD diagnosis can also lead to tailored educational support, such as Individual Education Plans (IEPs), to address specific learning needs.
Managing ADHD is an ongoing process. Regular follow-ups with a specialist will ensure treatment plans remain effective and are adjusted as needed.
