Functional endoscopic sinus surgery is a minimally invasive procedure used to address chronic sinus issues and other nasal complications that block airflow or drainage.

Mr Ali Qureishi, esteemed consultant ENT and rhinoplasty surgeon, explores the indications, procedural techniques, and outcomes of functional endoscopic sinus surgery.

What is functional endoscopic sinus surgery?

Functional endoscopic sinus surgery is a minimally invasive procedure designed to restore normal drainage and function to the sinus cavities.

It’s primarily used to treat chronic sinusitis when symptoms persist despite management with antibiotics, nasal sprays, or corticosteroids. Sinusitis can cause long-lasting nasal congestion, facial pain, and headaches.

Other conditions treated with functional endoscopic sinus surgery include nasal polyps, a deviated nasal septum, mucoceles, and fungal sinus infections.

How is functional endoscopic sinus surgery performed?

Functional endoscopic sinus surgery is performed using a thin, flexible tube called an endoscope, which is equipped with a light and camera to provide a detailed view of the nasal structures.

During the operation, your ENT surgeon will insert the endoscope through the nostrils to access the sinuses. Specialised instruments will then be used to remove obstructions, excise tissue, and reshape or widen the sinus openings.

The surgery is carried out under general anaesthesia to ensure your comfort. The duration can vary depending on the complexity of the case, but typically lasts between 1-3 hours.

For patients with multiple nasal or sinus issues, functional endoscopic sinus surgery can also be effectively combined with other nasal procedures, such as:

  • Septoplasty: Correction of a deviated nasal septum, which can further improve airflow and drainage.
  • Turbinate reduction: Shrinking or reshaping of swollen nasal turbinates to open up the airways.

What is the recovery time after functional endoscopic sinus surgery?

Unlike traditional sinus surgery, functional endoscopic sinus surgery doesn’t require external incisions. This approach reduces trauma to surrounding tissues, leading to shorter recovery times and lower risks of complications.

Mild discomfort, nasal congestion, or bleeding can occur in the initial days after surgery, but these symptoms typically resolve quickly. Prescribed antibiotics or steroids can help manage inflammation and prevent infection. Additionally, you should avoid irritants such as dust, smoke, and airborne particles during the healing process.

Most patients will return to work or normal daily activities within 1-2 weeks, although complete healing of the sinuses may take several months. Follow-up appointments with your surgeon are essential to monitor the healing of the sinuses and address any potential complications.

How successful is functional endoscopic sinus surgery?

Functional endoscopic sinus surgery has a high success rate and is considered one of the most effective treatments for chronic sinus conditions. Up to 90% of patients report significant symptom relief and an enhanced quality of life following the procedure.

However, it’s important to understand that functional endoscopic sinus surgery isn’t a permanent cure for all sinus problems. Underlying conditions like allergies or immune deficiencies will require ongoing medical supervision to prevent future sinus infections.

Regular follow-ups with your ENT surgeon will help monitor sinus health and address any recurring issues promptly.
