Breast reduction: Preparation, surgery and recovery
Breast reduction, also known as reduction mammaplasty, is a surgical procedure designed to remove excess tissue, fat, and skin from the breasts. For many people, this procedure can alleviate physical discomfort and lead to increased confidence and a better quality of life.
Mr Sarmad Tamimy, esteemed consultant plastic surgeon, provides an expert insight into the preparation, surgical process, and recovery after breast reduction.

How should I prepare for breast reduction?
Proper preparation is essential for a successful breast reduction. The first step requires detailed consultation with your plastic surgeon. During this meeting, your surgeon will review your medical history, discuss your goals, and perform a physical examination.
Before the surgery, your surgeon may recommend certain lifestyle changes. For instance, maintaining a stable weight is often advised, as significant weight changes post-surgery can alter the results. Furthermore, your surgeon may recommend stopping certain medications that increase bleeding risks.
If you smoke, you will need to quit smoking several weeks before and after the surgery to reduce the risk of complications. On the day of the surgery, ensure you have comfortable clothing, including a loose-fitting top and a supportive bra, as well as necessary supplies. It’s also important to plan for transportation home and assistance during the initial recovery period.
How is breast reduction performed?
Breast reduction is performed under general anaesthesia and typically lasts 2-5 hours, depending on the complexity of the case. The surgical technique used will depend on your anatomy and desired outcome. The most common techniques include:
- Anchor incision: This technique involves making incisions around the areola, vertically down to the breast crease, and horizontally along the crease. Used for larger reductions.
- Vertical or "lollipop" incision: This technique involves making incisions around the areola and a vertical line down to the breast crease. Used for moderate reductions.
- Doughnut incision: This technique involves making a circular incision around the areola. Used for smaller reductions or minor adjustments.
During the procedure, your surgeon will remove excess breast tissue, fat, and skin. The remaining breast tissue will then be reshaped to create a smaller, more proportionate contour. Subsequently, your surgeon will reposition the nipples and areolas to align with the new breast size, resulting in a natural and lifted appearance.
Breast reduction can often be combined with other procedures, such as a breast lift or liposuction in the surrounding areas to achieve a more contoured silhouette.
What is the recovery time after breast reduction?
Recovery times after breast reduction vary from person to person, but most patients can anticipate a healing period of 4-6 weeks.
Immediately after surgery, you will likely experience swelling, bruising, and discomfort, which can be managed with prescribed medications and the use of a surgical bra. While you may notice immediate relief from symptoms like back and shoulder pain, the final results of breast reduction will become more apparent as the swelling subsides.
Most patients will resume light activities, including work (if their job isn’t physically demanding), within 1-2 weeks. However, it’s crucial to avoid strenuous activities, heavy lifting, and exercise for at least 4-6 weeks to prevent strain on the healing tissues.
Will I have scars after breast reduction?
Scarring is an inevitable part of breast reduction, as incisions are necessary to remove excess tissue and reshape the breasts. However, your surgeon will use techniques to minimise scarring and ensure that scars are placed in less noticeable areas, such as around the areola or along the breast fold.
Initially, scars will appear red or raised, but they will gradually fade and flatten over time, becoming less noticeable within 1-2 years. To minimise scarring, follow your surgeon’s post-operative care instructions, avoiding sun exposure and using silicone scar treatment to help improve the appearance of the scars.
