Blepharoplasty, or eyelid surgery, is a specialised procedure to correct loose, puffy or sagging eyelids. It can be performed on the upper eyelids, lower eyelids or both at the same time. As well as cosmetic rejuvenation, blepharoplasty can address functional issues such as obstructed vision caused by drooping upper eyelid skin. The approach to surgery is increasingly personalised, using refined techniques that focus on natural, long-lasting results.


What does blepharoplasty involve?


Blepharoplasty is tailored to the individual’s needs. In some cases, it is combined with other procedures such as lower eyelid tightening (canthopexy), resurfacing of the skin with laser or chemical peels, or volume restoration using fat transfer. These combined approaches can help enhance the natural outcome, especially in cases where age-related changes involve more than just excess skin.


Upper eyelid surgery involves a fine incision in the eyelid crease to remove or reposition skin and fat. For the lower eyelid, incisions are usually made on the inside of the lid (transconjunctival) or just below the lash line (transcutaneous), allowing for discreet healing. The surgery typically takes between one to three hours, depending on complexity.



Preparing for surgery

 

Before surgery, patients have a detailed consultation where concerns and goals are discussed. Reviewing previous photos, such as those from a person’s 20s, may help guide the surgical plan.


In the weeks leading up to surgery, patients are advised to avoid certain medications, supplements, smoking and alcohol, all of which can affect healing. Preparation also includes arranging transport home, creating a comfortable recovery area, and following specific guidance on nutrition and herbal remedies to minimise bruising and swelling. Clear communication about expectations and a thorough assessment of overall health and lifestyle factors are essential.



How is the procedure carried out?

 

The procedure is commonly performed under sedation anaesthesia, which offers quicker recovery than general anaesthesia. Most patients are alert and comfortable within 30 minutes post-surgery. During surgery, careful adjustments are made to remove or reposition skin, fat or muscle as needed. Advanced techniques aim to preserve or restore volume for a more youthful appearance.


Additional treatments, such as skin resurfacing or regenerative medicine, may be used to enhance the outcome. Incisions are closed using fine sutures or surgical adhesives, and care is taken to position them in areas that heal inconspicuously.



Recovery and aftercare

 

Recovery from blepharoplasty is typically smooth. Swelling and bruising are expected for the first week or two but gradually resolve. Patients are advised to rest for the first two weeks, and can often return to work and light activity after 10–14 days. Strenuous exercise should be avoided for around four weeks.


Regular follow-up is key. Daily check-ins during the early healing phase, along with in-person assessments, help monitor progress and provide reassurance. Stitches are often removed within the first week.



How long do results last?

 

A customised blepharoplasty can deliver natural-looking results that may last 10 years or more, depending on individual ageing and lifestyle factors. For many, the outcome is not only aesthetic but also functional, improving visual comfort and confidence.
