SMILE laser eye surgery (Small Incision Lenticule Extraction) is a surgical technique to correct myopia and astigmatism, performed by Mr Radwan Almousa at the My-iClinic

SMILE laser eye surgery differs from the LASIK procedure as the corneal incision is very small (2-3 mm) and the tissue is taken from the mid-corneal stroma leaving behind the strong anterior corneal tissue, which helps preserve the corneal strength.

What is the SMILE laser eye surgery procedure?

SMILE is a laser surgical technique that involves the removal of a refractive lenticule (lens) through a small incision that is only 2-3 mm wide.

Unlike LASIK, SMILE does not require excimer laser photo-ablation or a full flap cut. Therefore, SMILE is considered a minimally invasive approach to corneal refractive surgery.

SMILE laser eye surgery is gaining popularity over flap-based corneal ablative procedures due to its lower adverse effects on the ocular surface and reduced incidence of dry eyes.

The elimination of the corneal flap results in greater biomechanical stability and a reduced risk of flap-related complications.

Is SMILE laser eye surgery safe?

While every surgical procedure carries some degree of risk, SMILE is considered a flap-less procedure, which means it excludes all complications associated with the corneal flap in LASIK.

However, SMILE is not entirely immune to complications. The most common complication is lenticule misdissection, which can lead to the procedure being stopped.

However, advancements in lenticule dissection techniques are making this complication less frequent.

Difficult lenticule extraction is another potential complication that may occur in approximately 9% of cases. This is often related to laser energy settings and can result in longer surgery times and delayed visual recovery due to corneal oedema.Fortunately, this complication is becoming less common as energy settings continue to improve.

Cap tears are also possible, occurring in approximately 1% of cases. This happens when the anterior part of the cornea overlying the lenticule (the cap) tears, making it difficult to remove the lenticule.

If this complication occurs, a bandage contact lens can be used to speed up the healing process, and while visual recovery may be delayed for up to a month, the final visual outcome is typically no worse than that of an uneventful surgery.

Retained lenticule is another rare complication that may occur in around 3% of cases. This occurs when the lenticule is not completely removed due to difficult lenticule dissection, resulting in suboptimal visual outcomes. If this happens, further refractive surgery, such as LASIK, can be performed three months later, with a very good outcome.

The recurrence of short-sightedness is another possible complication, with a reported incidence of 2.2% to 2.9% after SMILE, which is comparable or slightly lower than that reported for LASIK.

Enhancements can be achieved with further refractive surgeries such as LASIK or PRK.

While most SMILE complications are related to difficult lenticule extraction, these complications are becoming less common with further improvement of the laser settings. Even if complications do occur, the visual outcomes are still typically excellent, albeit with a delayed recovery period.

How long does recovery take after surgery?

On the first day after undergoing SMILE laser eye surgery, approximately 55% of patients can expect to achieve 20/20 vision.

This percentage is expected to increase to 80% after one week, 93% after one month, and 96% after three months.

It is common for patients to experience reduced quality of vision, such as glare and haloes, during the first day after the procedure due to postoperative corneal swelling.

As the swelling subsides, the quality of vision should improve, and over the following three months, there may be further improvement in visual acuity.
